The response to incidents, errors and other issues often lingers in symptom management. The underlying cause or causes remain untouched, resulting in a high probability that the same incidents will recur.
If you want your organization to learn from mistakes, you need clarity about the underlying cause or causes of the mistakes. The root cause analysis is an instrument that provides this. It is a systematic, unbiased and multidisciplinary search for the why of the why. With its results, you can implement effective interventions, interventions that can prevent repetition.
