Mistakes are there to learn from. Based on the scientific research of Muel Kaptein, KPMG developed a methodology for root cause analysis (RCA), which answers the question why issues keep repeating themselves.

The response to incidents, errors and other issues often lingers in symptom management. The underlying cause or causes remain untouched, resulting in a high probability that the same incidents will recur.

If you want your organization to learn from mistakes, you need clarity about the underlying cause or causes of the mistakes. The root cause analysis is an instrument that provides this. It is a systematic, unbiased and multidisciplinary search for the why of the why. With its results, you can implement effective interventions, interventions that can prevent repetition.

KPMG is considering this RCA as an integral part of control, compliance and internal audit. Its application is tailor-made and to this end we offer multiple services, from performing analyses to implementing, monitoring and supporting a RCA structure in the organisation. We have more than twenty years of experience with the Soft Controls methodology incorporated in the RCA. In recent years, we have also gained extensive experience with the RCA in the financial sector, at large companies and in the public sector.
